Vukadinovic and Mantle [19] assessed a sample of 15 patients with delusion of control. A cut-off score of > or =5 out of 10 (based on the Working Alliance Inventory scale) was used to differentiate control from patients with diagnosed schizophrenia. Four dimensions were assessed (interpersonal effectiveness, goal and task congruence, goal and task satisfaction and satisfaction with the alliance) and their scores were associated with the degree of insight. These results are inconsistent with the present study in which we did not find any differences between delusional types and distribution of insight. For this reason, the authors used a cut-off score of > or =17 for delusional control and found a difference in interpersonal effectiveness, goal and task congruence and satisfaction with the alliance between the two groups. This was a small sample study (only 15 patients with delusion of control) and data has to be confirmed by future studies.
In this study, delusional disorder was associated with family history of any psychiatric disorder. In particular, patients with primary delusional disorder had a threefold increase in family history of major depression, twofold increase in family history of delusional disorder, and eightfold increase in family history of schizophrenia. (The absolute risk for primary delusional disorder is about 1.6%, as family history for any psychiatric disorder is about 18.3% and for schizophrenia is about 4.1%). We have not found any difference between delusional types in the prevalence of secondary psychiatric disorders. Regarding the association between family history and delusional disorder, the most recent literature suggests that delusional disorder may be the result of a genetic vulnerability that predisposes to the development of psychotic symptoms in certain situations [21].
A recent study [20] on the prevalence of delusional disorder among first degree relatives of schizophrenia patients detected a prevalence of 1.3% (95% confidence interval, 0.9-1.8). In this study, patients were interviewed about previous psychiatric problems and relatives were interviewed about a number of relevant characteristics. The authors concluded that the frequency of delusional disorder among first-degree relatives of schizophrenia patients is lower than that reported in other studies, and that the study design may explain the low rate of diagnosing. However, this is the first family study on this topic and further prospective studies are needed to confirm the results. This study was carried out with a small sample size and data are only available for a subsample (all relatives were interviewed, 13 patients with a diagnosis of delusional disorder and their relatives were interviewed).
